BEFORE YOUR APPOINTMENT

After scheduling your first appointment, you will receive electronic forms by email.

Completing these forms in advance helps your doctor better understand:

✔ Your primary concern

✔ When and how your symptoms began

✔ Relevant medical history

✔ Previous treatments

✔ Activities affected by your condition

✔ Your insurance or claim information

✔ Your treatment goals

What to Bring

Please bring the following items to your first appointment:

  • Photo identification

  • Insurance card(s)

  • Relevant medical records or imaging reports (if available)

  • Auto accident or workers' compensation claim information (if applicable)

What to Wear

We recommend wearing comfortable clothing that allows you to move easily and helps your doctor evaluate the area of concern.

YOUR FIRST APPOINTMENT

New-patient appointments are generally scheduled for approximately 45 minutes, allowing time to discuss your concerns, perform a thorough examination, and review treatment options.

Conversation and History

Your doctor will take time to learn about your symptoms, health history, daily activities, work, exercise, previous injuries, and personal goals.

Examination

Your examination may include range-of-motion testing, orthopedic and neurological assessments, muscle strength testing, palpation, and functional movement screening. These tests help us better understand your condition and identify areas that may need attention.

Explanation of Findings

Following your examination, your doctor will discuss:

✔ Important examination findings

✔ Your likely diagnosis and contributing factors

✔ Whether chiropractic care is appropriate for your condition

✔ Whether imaging or referral may be necessary

✔ Recommended treatment options

✔ Goals for recovery and long-term improvement

Treatment

When appropriate, treatment may begin during your first appointment. This may include chiropractic adjustments, manual therapy, or other techniques based on your individual needs and examination findings.

Your Treatment Plan

If ongoing care is recommended, your doctor will explain:

  • Recommended visit frequency

  • Expected duration of your initial treatment plan

  • Chiropractic and rehabilitative treatment options

  • Home exercises and activity recommendations

  • Functional goals and expected progress

  • When your progress will be reassessed

Our goal is not only to help relieve your symptoms but also to improve movement, restore function, and support your long-term health.

FOLLOW-UP VISITS

Regular follow-up appointments are generally scheduled for approximately 15 minutes.

Depending on your condition and progress, your visit may include:

✔ Review of your symptoms and progress

✔ Focused reassessment

✔ Chiropractic adjustments

✔ Soft-tissue and manual therapy

✔ Therapeutic exercises

✔ Home exercise recommendations

✔ Guidance on activity and injury prevention

Each visit is tailored to your needs, so treatment may vary as you progress.

RE-EXAMINATIONS

Re-examinations are generally scheduled for approximately 30 minutes.

These appointments allow us to evaluate your progress and determine whether your treatment plan should be adjusted.

We will assess:

✔ Improvements in pain, mobility, and function

✔ Progress toward your treatment goals

✔ Whether visit frequency should change

✔ Whether additional treatment may be beneficial

✔ Whether further evaluation or referral is appropriate

✔ Your readiness to transition toward independent management and preventive care
